It was an honour to be part of the SMISS-AP (Society for Minimally Invasive Spine Surgery – Asia Pacific) Conference held at the iconic Taj Mahal Palace, Mumbai.

I had the opportunity to present on “Risk Factors for Infection After Spine Surgery” — focusing on identifying patient- and procedure-related risk factors, reviewing predictive models, and discussing evidence-based strategies to mitigate surgical site infections.

Also participated as a panelist in OLIF sessions, engaging in stimulating discussions with esteemed colleagues from across the Asia-Pacific region.

A special highlight was listening to Mr. Gautam Adani, the Chief Guest. His thoughts on perseverance, innovation, and India's role in the global economy were truly inspiring.

Thank you to the organizing team Dr Ayush Sharma and Dr Arvind Kulkarni for putting together such a dynamic and well-structured academic event.

🔹 Vertebral Fracture with Vacuum Cleft – Treated with Transpedicular Bone Grafting & Minimally Invasive Fixation 🔹

This 72 year old lady had a painful vertebral fracture with a vacuum cleft — a sign of non-union often seen in osteoporotic or avascular vertebrae (Kümmell’s disease). Apart from pain, she couldn't stand or walk for long and had frequent falls.

Instead of using cement alone, we chose a more biological and durable solution: ✅ Transpedicular bone grafting with
Minimally Invasive Pedicle Screw Fixation

🦴 Why bone grafting?
Unlike vertebroplasty, which fills the cleft with cement, bone grafting promotes natural healing
• Avoids cement-related complications

• Encourages long-term bone remodeling

The 10 month follow-up radiograph shows fracture healing. Patient is pain-free and is able to walk independently.

Vertebroplasty related complications include cement embolization and cement migration into the canal as well as adjacent fractures. These can be avoided with Transpedicular bone grafting. However Transpedicular bone grafting takes time to heal. Pain relief is not immediate as with Vertebroplasty.
