Today, I was revisiting and re-reading the basics of ECG, and I had Just an important realization.❤️‍🩹

While we often focus heavily on interpreting the ECG itself, we tend to overlook essential aspects like considering patient history and comorbidities, and maintaining proper patient positioning, correct lead placement, understanding the ECG machine basics, ensuring good contact with leads etc.

These factors are just as critical for accurate ECG readings.

For example, misplacing V1 and V2 (in the 2nd or 3rd intercostal space instead of the 4th) can significantly alter ECG results, leading to false interpretations. It can cause abnormal P waves (like a negative P in V1), mimic incomplete right bundle branch block (IRBBB), or even create false Q waves that resemble a septal MI. Such misplacements can also trigger false alerts for STEMI or Brugada patterns, especially in young, healthy individuals.

To truly master ECG interpretation, we must consider all aspects of the process—not just the readings themselves. Proper lead placement and a comprehensive approach are essential to avoid misdiagnoses and ensure accurate results, especially when assessing conditions like pulmonary embolism or ischemia.

For accurate ECG readings, consider proper lead placement, clean skin, good electrode contact, patient posture, and minimizing movement. Ensure the ECG machine is calibrated, free from electrical interference, and maintain consistent electrode placement. Factor in the patient’s age, body type, heart rate, and comorbidities for reliable results and accurate interpretation.

A KEY LEARNING From my Medical Training.🔑❤️

One crucial lesson I’ve learned during my training is that ANY DISEASE CAN AFFECT ANY ORGAN or SYSTEM of the body.
I learnt this insight from my competent supervisor, Dr. Sheraz Jamal, and it truly reshaped my understanding of diagnosis.

Before starting my house job and clinical training, I had the misconception that specific diseases would only affect specific organs or systems. However, the reality is quite different.

When making a diagnosis, it's essential to remember that infections or diseases can impact ANY system, not just the one you might expect. Symptoms can present in a way that's typical, but they can also be ATYPICAL, sometimes affecting organs or systems you wouldn't immediately associate with the disease based on textbook knowledge.
For example,

COVID-19. While the disease is primarily known for causing respiratory symptoms, such as cough and shortness of breath, it can also affect a wide range of other systems in the body.
i.e
•CNS: Confusion, dizziness, or even strokes, which are not the first symptoms you'd associate with a respiratory virus.
•GIT: Nausea, vomiting, diarrhea, or abdominal pain, which could easily be mistaken for a gastrointestinal infection, rather than something linked to the lungs.
• CVS: Myocarditis (inflammation of the heart muscle) or arrhythmias in some patients, even those with no pre-existing heart conditions.
And many more…

This is why it's critical to think outside the box, maintain a broad differential diagnosis, and stay open-minded in your clinical approach.

As healthcare professionals (especially Doctors of Internal Medicine), we must always be prepared to explore all possibilities and not limit ourselves to conventional patterns. The complexity of the human body requires us to constantly question, learn, and adapt.

• I sometimes get lost and feel low because of my this thought, that why my journey of Social Media is taking so much time, to be on the top
BUT
Then experiences like these make me feel comfortable and at ease that yes, we are adding something right and beneficial to the society, by the will of ALLAH.

Like yesterday I was at workshop at Khyber Teaching Hospital Peshawar and one junior medical fellow during workshop was smiling at me. After workshop he came to me and shared that my video ‘PGMI test: My Experience’ had a significant impact on him, helping him secure high marks.

I can’t describe how it felt !
Such feelings of being right and beneficial to others itself is like TO BE ON THE TOP.

ہمارے جاہلی معاشرے کا ایک المیہ یہ بھی ہے کہ ہم میں سے جس بندے پر اللہ رزق کشادہ کر دیتا ہے ، (چاہے وہ مال و دولت یا گھر یا گاڑی میں ہو، حسن یا بیوی یا خاندان میں ہو، علم میں ہو، قابلیت میں ہو، دین میں ہو)

تو باقی لوگ اس سے جلنا شروع کر دیتے ہیں۔

حسد ، بغض ، کینہ پروری، دشمنی، بہتان و الزام تراشی اور حتی کہ جادو ٹونے تک لوگ گر جاتے ہیں، مطلب اپنے ایمان سے بھی دوسروں سے حسد میں ہاتھ دھو
بیٹھتے ہیں۔

اس کا حل یہ ہے کہ ذہنی و مالی پسماندہ لوگوں سے دور ہی رہا جائے، اللہ سے حفاظت کی دعائیں کی جائے، لوگوں کو اپنی نعمتیں زیادہ نہ دکھائی جائے اور کم از کم خود ایسا نہ بنا جائے۔۔

اللہ ہر جگہ، ہر قدم اور ہر قسم کے شر اور شریروں سے ہماری حفاظت فرمائیں۔
