Interestingly, th concrete slab is on the other side furthest away from the launch pad. I think it might have been a piece that hit the tall vertical storage tanks denting one of them and then it bounced away and hit the He tanks.

Being in the concrete business for over 70 years (pouring with my dad) this part of the project was a terrible failure. The design team used a terrible model for their forces which didn’t model the PULSATING forces correctly or the model or advice on concrete performance was solely lacking. Even today, concrete requires huge experience as well as modelling. I suspect the experience was lacking.
