The cash incentive i would offer is this. The NHS won't treat obesity related illnesses for free. If you choose obesity. You pay for the consequences.

I'm obese and have been eating one meal a day for the past three years, because of a medical condition I cannot eat more than one meal a day. So for people assuming obese people eat lots isn't factual.

Another β€˜solution’ that punishes the people who do the right thing and take care of themselves. If it’s about saving the NHS money, perhaps a small stipend for people who meet certain parameters for being in exceptional physical shape. That’s an incentive for everybody to get healthier, and it doesn’t cut out the people who are actually taking care of themselves.
