I imagine it depends on a number of factors. How frequently you wash, how dirty you were going in, how new the soap is, how aggressively you're using it, how good quality it is, the quality of the water, and probably some others I can't quite think of.

Yes Practically maybe because it depends on if the soap is like anti-bacterial how hot the water is, the amount of dirt/stanky stuff on that area and what area, because someone could be burning alive killing EVERY single germ on their body in the process

No, you body doesn’t suddenly become dirty because you aren’t actively scrubbing it with soap. It takes time to start sweating
