Uniball Vision are the best writing utensil I've ever used. My current favorite size is the micro at .5mm but they make the 1mm, .7mm, and a .35mm as well.

My preference for pens for writing, and also black-lining sketches, is a uni Pin Fine Line pen, tip size 0.05, water and fade proof, pigment ink pen. I think I go through about 5 or 6 of them in a month.

It depends a bit on the circumstances. I'd love to use a fountain pen more often, but they tend to be less reliable and more fallible than a ballpoint, especially in terms of issues like ink running or being able to rely upon them on short notice.

It totally depends on the medium, writing on paper or wood or plasterboard or plastic?

While I picked "Ballpoint" my true answer is dependant on what I'm doing. If I'm writing notes, then "Ballpoint" but if I'm sketching or drawing then start with pencil (most likely mechanical) and if I need it finalized then felt tip pen overtop of that.

.5mm Mechanical pencil, is my first choice. But when I have to use ink, I like the Bic velocity ballpoint pens. They're just so smooth
