My answers look good, but I travel 2.5 hours there and 2.5 hours back to see my ace-affirming GP as opposed to e.g. someone at the GP office that I could walk to in 5-10 minutes. Note: I'm (very) in favour of this questionnaire series plan, and I agree one has to start somewhere. 5 questions seems a reasonable place to start.
