I know the blue face might not have been intentional, but I kinda see it as if Sir Pentious was to give up on trying to be good in Heaven and relapse into his old, moustache-twirling villany ways. He's wearing his old outfit and has somehow psychologically gotten his body to return his pink eyes and eye hood.
