I was diagnosed with Degenerative Disc Disease at 17 years old, but my spine looked as though I was 40 years old. 🫣

After this diagnosis (along with stenosis, spondy, sciatica, and SI joint dysfunction), I went on a deep dive in order to heal my body, so I could get back to doing what I love, surfing. πŸ„

This dive even led me to my πŸ₯Ό Doctorate in Physical Therapy in 2015, but that still didn’t heal my pain. It wasn’t until I stopped the causes of my pain and relearned a new relationship with my body, one that moves from my core, that I was able to live a normal life. βš“

DDD is a common diagnosis for those with chronic lower back pain. DDD is defined as accelerated spine degeneration, resulting in disc thinning and loss of fluid, often influenced by lifestyle, posture, and muscular imbalances.

While this is a normal part of aging, a DDD diagnosis at a young age may indicate accelerated degeneration.

Spinal decompression or traction is the separation of the spinal vertebrae to create space for the discs to imbibe fluid and alleviate pressure. 😌

There are various methods of spinal decompression like inversion tables, traction devices in physical therapy clinics, and hanging exercises.Β 

While these methods are popular, they may stretch and potentially worsen compromised tissues such as discs and ligaments. 🩻

Spinal decompression can also alleviate nerve compression, but the relief is short-term, typically lasting only a few hours. ⏳

Nerve roots exit your spinal cord, run through your sacrum then right behind your piriformis muscle! 😲

This makes the sciatic nerve vulnerable to compression ⚑ if the piriformis is chronically TIGHT, which is labeled piriformis syndrome.

The piriformis muscle is 1 of only 2 muscles that stabilize the SI joint by attaching to the sacrum instead of the pelvis (gluteus maximus is the other).

πŸ‘‰ So why is the piriformis muscle chronically TIGHT, pressing on the sciatic nerve??Β 

In the predictable pattern of muscle imbalances, the glutes are inhibited and too weak. This destabilizes the pelvis, and the deeper hip muscles like the piriformis must compensate by doing the only thing it knows to do, contract. That presses on the sciatic nerve!

βœ… Solution: πŸ›‘ STOP stretching your tight piriformis! It’s contracting for a reason: to protect. Instead give your piriformis the support it needs so it can feel safe to let go, by strengthening the hip complex.Β πŸ‘

The QL is the deepest back muscle and it attaches to the bottom of the rib cage, the lumbar spine, and then to the top of the pelvis. It’s located right next to your hip flexors, the iliopsoas.

It’s function is to help you side bend and extend your spine, like a back bend. Compared to the other muscles around it, it doesn't have much force and isn’t responsible for that much movement. The erector spinae muscles are 10x stronger than the QL during spinal extension. πŸ’ͺ

Since the QL is so deep and comparatively not that strong, it's difficult to target with exercise. πŸ€”

πŸ‘‰ So WHY are we told a tight QL is the cause of back pain?? ⁉️

πŸ‘‰And why is the QL so chronically tight for so many people? ⁉️

ANSWER: The QL is most likely compensating for your spine being unstable. It compresses your spine and discs to mimic stability. Since it helps with spinal extension and it’s located in the back, it’s tightness is contributing to anterior pelvic tilt.

πŸ—¨οΈ The muscle is tight so I should stretch it right? NO!!!! ❌😯 The muscle is tight for a very important reason: to protect your unstable spine!

βœ… The SOLUTION to a tight QL is to increase spinal stability through intra-abdominal pressure (IAP) so the QL doesn’t need to compensate. Then the QL will feel safe to relax.
