Differentiating Points Pediatric Nephrotic/Nephritic Syndromes

1. Minimal Change Disease (MCD)
👉 Most common in children
👉 Nephrotic syndrome: heavy proteinuria, edema, hypoalbuminemia, hyperlipidemia
👉 No hematuria, normal BP
👉 Light microscopy normal, EM shows podocyte effacement
👉 Steroid responsive


2. Focal Segmental Glomerulosclerosis (FSGS)
👉 Nephrotic syndrome, but often steroid-resistant
👉 Common in adolescents
👉 Light microscopy: focal & segmental sclerosis
👉 Poor prognosis → progresses to CKD


3. IgA Nephropathy (Berger’s Disease)
👉 Recurrent hematuria, often after URI/GI infection (synpharyngitic)
👉 Usually nephritic, not nephrotic
👉 IgA deposits on immunofluorescence


4. Post-Streptococcal Glomerulonephritis (PSGN)
👉 1–3 weeks post streptococcal infection
👉 Cola-colored urine (gross hematuria), edema, hypertension
👉 Nephritic syndrome
👉 Low C3, "humps" on EM (subepithelial deposits)
