A 39-year-old man presents with progressive fatigue, intermittent fever, nonproductive cough, and increasing shortness of breath for the past 2 weeks. He reports significant unintentional weight loss over the last year along with recurrent oral ulcers and chronic diarrhea. He was treated multiple times in the past for respiratory infections.
On examination, temperature is 39°C (102.2°F), respiratory rate is 36/min, and oxygen saturation is reduced on room air. Bilateral lung auscultation reveals minimal crackles despite marked dyspnea. Multiple non-tender cervical and axillary lymph nodes are palpable. Oral examination shows white plaques over the tongue that can be scraped off.
Laboratory studies show:
Hb: 9.8 g/dL
Total leukocyte count: 3200/mm³
CD4+ T-cell count: 170/mm³
Chest radiograph demonstrates diffuse bilateral interstitial and perihilar infiltrates without focal lobar consolidation or cavitation. Bronchoalveolar lavage fluid stained with Wright-Giemsa and silver stain reveals multiple round to cup-shaped cystic structures. Which of the following is the most likely diagnosis?
