Wow! It took some real practice, but I managed to beat this game on Ultra-Nightmare! Stressful, for sure, but extremely fun and rewarding. Hardest parts are definitely Cultist Base and the Gladiator boss fight for me. If anyone wants to beat UN before The Dark Ages releases, here's three tips:

1. ALWAYS replenish armor even if you're full. Use the Flame Belch constantly; you'll want to keep it on cooldown as much as possible. Make sure you're eliminating fodder with the flame Meathook as well throughout any fight. It is invaluable to keep your armor reserves as high as you can during gameplay.
2. Don't be discouraged about dying. Instead, remember HOW you died and formulate a strategy that avoids that situation the next time.
3. Focus on pressure units like Mancubi, Cacodemons, Pain Elementals, and Arachnotrons before engaging any other demon. They can get you down to a "Low Health" screen faster than anything else.
4. Whiplashes and Dread Knights are consistently dangerous through the campaign, but one strategy can kill them in moments. Microwave beam stun for up to 2 seconds, then dash in for a blood punch, followed by a rocket or some shotgun shots.
5. Avoid LOW HEALTH screens like the plague. You never want to be in a situation where panic blinds your gameplay. I'm not perfect at avoiding these, but practicing Ultra-Nightmare has helped me to reduce the appearance of this event much more. Use the ice bomb, blood punch, or the Crucible if you're ever in a bad spot.
