Confusion, blurred vision, anion gap metabolic acidosis, history of alcohol use = Methanol toxicity

Vomiting, watery diarrhea, garlic breath after exposure to pesticides = arsenic toxicity

Confusion, flank pain, hematuria, anion gap metabolic acidosis, and calcium oxalate urine crystals = Ethylene glycol toxicity

Unconscious patient with respiratory rate of 6 and miosis = Opioid overdose (treat with naloxone)

Patient with miosis, bradycardia, diarrhea, sweating profusely after nerve gas attack = Acetylcholinesterase inhibitor poisoning (AKA organophosphates: treat with atropine first!)

NaHCO3 works for: Barbiturates/ASA/TCA
Amphetamine = Ammonium chloride & benzo
Jimson weed / nightshade = Physostigmine
Benzo = Flumazenil

Dabigatran = iDarucizumab

Differentiating CO/Cyanide/Methemoglobinemia

-Cyanide = normal PaO2 / O2 Sat / A-a gradient

-CO = high O2 Sat, low PaO2 (CO outcompetes O2)

-Methemoglobinemia = O2 Sat @ 80-85%, low/normal PaO2 (but there will be > 5% gap between O2 sat and PaO2!)
