Here I share a challenging but highly rewarding endovascular procedure on a 79-year-old male with an extensive medical history including:
* Cardiac amyloidosis (TTR wild-type) with heart failure and preserved ejection fraction
* Atrial fibrillation/flutter on ACOD
* Prior ischemic heart disease (NSTE-ACS 2020) and right MCA stroke (2020)
* Type 2 diabetes, dyslipidemia, hyperuricemia
* Stage 3b chronic kidney disease (baseline creatinine 2.3–2.6 mg/dL)
* Previous failed attempt at popliteal recanalization (Dec 2025) and upper extremity embolism
The patient was admitted for planned recanalization of a chronic occlusion in the third portion of the left popliteal artery, with bypass to the anterior tibial artery as backup.

💥 New case!

64-year-old male patient with CLTI — R4/5 — in the right lower limb.

👀 Previously treated twice at another center

In 2019: bilateral common iliac artery stenting + iliac stent with BeGraft

In 2025: new intervention for right-sided axis thrombosis → bilateral femoral endarterectomy + kissing stent technique using 7 mm VBX (flared to 10 mm) and 7mm Viabahn.

🙈 Unfortunately, only 6 months later, he presents again with re-thrombosis of the right axis.

❗ Attached is the pre-operative angio-CT reconstruction showing the current anatomy.

Clinical context:

Active working male, <65 years old

High functional demand

Recurrent failure of endovascular reconstruction

What would be 🫵 treatment strategy in this re-do case?

Open surgery? Hybrid approach? Advanced endovascular techniques (new device selection, atherectomy, drug-eluting tech, etc.)? Different kissing configuration?

Hi everybody, in my next video I show another BTK revascularization.

It was needed a retrograde access. Due to high calcification, US distal puncture was not easy, so I performed it by angiographic guidance.

As you can see through my other videos, I think that drug eluting balloons are also applicable for BTK and BTA vessels so in this case, after PTA with non compliant JADE balloon, Selution SLR (cordis) was used as DEB in ATA and Peroneal artery.
