A 56-year-old British man presents with a deformity of his right ring finger, which has gradually developed over a year. He says that for the past two months, the finger has been getting in the way while combing hair or placing his hand into a trouser pocket. He has noticed hardening of the palmar skin close to the base of the right ring finger. He has diabetes, well-controlled on oral medication, for the past 7 years. There is no history of injury to his right hand.

On examination of his right hand, there is a firm subcutaneous nodule on the distal-lateral aspect of palm. A thick fibrous cord measuring 2 cm is palpable proximal to the 4th metacarpophalangeal joint. There is no tenderness. The fourth digit has flexion deformity and cannot be extended completely. Other digits and sensory examination are within normal limits.

What is the most likely diagnosis?
