→ Will I experience herxing after Lymph Drainage treatments?

Q: Clients often ask whether after Lymph Drainage Massage they will feel herxing, which is that feeling of being sick with flu like symptoms, especially when Lyme disease, autoimmune conditions, or lymphatic congestion are involved.

A: Herxing is not the goal of proper Lymph Drainage. At National Lymphatic Centers, treatment always begins at the venous angle and terminus, never at the feet. Drainage starts in small, controlled areas such as the clavicle, sternum, axillary underarms, head and cranium, then progresses in a precise sequence through the arms, thoracic region, abdomen, and legs.

This orderly approach allows lymphatic load to be processed gradually through the lymph nodes, preventing inflammation from being pushed into circulation all at once. The objective is clean, controlled drainage—moving debris to the dustpan, not creating a dust cloud. This method is part of the trademarked Vogel Method of MLD, which is the NLC standard at all of our locations, established in 1991.

→ Why does my arm not drain even while wearing a compression sleeve?

Q: A common question we receive is why swelling persists even with compression.

A: One of the definitions of Lymphedema is that the lymph fluid cannot get out for one reason or another. If an arm is not draining, the issue may not be the sleeve itself. The sleeve may be too tight, or the limb and shoulder region may still be congested and require proper drainage first.

Compression can only work when lymph fluid has a clear pathway. In many cases, the arm needs to be manually drained before a sleeve can be effective. If a sleeve leaves deep red marks near the shoulder, that is often a sign of congestion rather than support.

Always be measured and fitted by a Certified Lymphedema Therapist (CLT) to ensure compression supports lymph flow rather than restricting it.

Q. When it comes to post-op recovery, many people call for consults and ask: "How soon can I start my massages after surgery?"

A. The answer depends on your individual healing process and your surgeon’s post-operative protocol. As a general guide, therapy may begin around 5 days after liposuction. For abdominoplasty (tummy tuck), it is often closer to 10 days post-op, depending on healing and clearance. For oncology-related surgeries, such as mastectomy, more rest time may be needed before beginning therapy.

Q: A common question we receive is: Can individuals with breathing conditions safely receive Manual Lymphatic Drainage (MLD)?

A: For someone with a respiratory condition, visiting a hospital lymphedema clinic may be safest. First ask your doctor and have them fax a script that MLD manual lymphatic drainage is safe for you. So, the answer is customized for each individual patient, with careful consideration. MLD can actually support those with breathing challenges by reducing fluid congestion and encouraging gentle diaphragmatic movement. However, each case is unique. NLC does not see patients with AFIB, COPD, emphysema, chronic asthma, shortness of breath or other respiratory issues. Instead, a good resource would be an in-hospital clinic.
