As doctors, we often see how quietly serious conditions can be missed, especially in young patients.
This patient was only 18 years old. She had been experiencing progressive abdominal distension, pain, and recurrent vomiting for quite some time. These symptoms were initially assumed to be simple bloating, and detailed evaluation was delayed.
By the time she reached us, imaging revealed a giant ovarian cyst measuring nearly 35 cm. At this size, the concern extends beyond discomfort to the risk of malignancy, ovarian torsion, cyst rupture, and permanent damage to the ovary.
This was a difficult and delayed presentation, requiring careful surgical planning. At her age, preserving fertility was a key priority alongside providing safe and complete treatment.
During surgery, nearly 2 litres of thick, dark green-brown fluid was drained from the cyst. Careful inspection showed no cancer, no nodules, no ascites, and no suspicious growths. It was a benign ovarian cyst that had grown silently over time.
A fertility-preserving ovarian cystectomy was successfully performed. The ovary was saved, significantly improving her chances of normal ovarian function and future fertility.
Cases like this highlight the importance of early evaluation of persistent abdominal symptoms, especially in adolescents and young women. What may seem harmless initially can, if missed, threaten both health and reproductive potential.
