GOLD Classification of COPD
GOLD (Global Initiative for Chronic Obstructive Lung Disease) classification helps assess the severity of airflow limitation in COPD based on spirometry (PFT) results. Severity of COPD can be assisted by this.
It mainly uses FEV₁ % predicted after bronchodilator.
1. GOLD Grades (Airflow Limitation Severity)
Based on FEV₁ % predicted:
GOLD 1 – Mild
FEV₁ ≥ 80% predicted
Symptoms usually mild, occasional cough
GOLD 2 – Moderate
FEV₁ 50–79% predicted
Shortness of breath on exertion, persistent cough
GOLD 3 – Severe
FEV₁ 30–49% predicted
Increased breathlessness, frequent exacerbations
GOLD 4 – Very Severe
FEV₁ < 30% predicted
Severe symptoms, risk of respiratory failure
