DEAR COLLEAGUES, DIRECTORS OF DIAGNOSIS AND TREATMENT PLANNING, ENDODONTIC EDUCATORS, AND AEGD PROGRAM FACULTY,

I am Marwan Abou-Rass, Professor Emeritus of Endodontics at the University of Southern California and founder of the AbouRass Endodontics Academy.

Having spent over fifty years in endodontic and advanced general dentistry education. I remain convinced that our priority in clinical education should be teaching students to think diagnostically before they start treatment.

This philosophy led to the development of the 4R Operational Diagnosis (4ROD) Protocol, an evidence-based framework integrating 4 essential diagnostic domains:
R1 — Patient Report & Rapport
www.genesispub.org/resource/images/articles/pdf755…
R2 — Radiographic Findings & Interpretation
manuscriptscientific.com/images/2ddd2976aac44b31e5…
R3 — Response Testing
manuscriptscientific.com/images/17fd69979925a4b1b5…
R4 — Restorative & Tooth Structure Assessment
manuscriptscientific.com/images/7e750d1dcbcf67c8dd…

Although developed from an endodontic perspective, 4ROD extends beyond traditional pulpal and periapical diagnosis by incorporating the related subjects in periodontics, restorative, prosthodontics, and orthodontics into the diagnostic process. This approach encourages multidisciplinary clinical reasoning while maintaining endodontic diagnosis as its clinical foundation.

Because you help teach future dentists how to think before deciding how to treat, I would like to offer diagnosis and treatment-planning educators, endodontic faculty, and AEGD educators complimentary access to the AbouRass Endodontics Academy to evaluate the educational program firsthand.

There is no obligation to subscribe to or promote the Academy. I simply welcome your professional assessment and, where appropriate, opportunities for educational collaboration.

For complimentary faculty access, please contact mar@abourass.com

If another faculty member at your institution is responsible for diagnosis and treatment planning, I would greatly appreciate your forwarding this invitation to that colleague.

ANNOUNCING THE ABOURASS ENDODONTICS ACADEMY INTERNATIONAL STUDENT AMBASSADOR PROGRAM

Dental education should extend beyond the dental school walls.
The AbouRass Endodontics Academy is launching an international initiative to connect motivated dental students interested in evidence-based diagnosis, diagnostic clinical reasoning, and the diagnostic aspects of comprehensive patient care.

The Student Ambassador Program will bring together students from dental schools worldwide who want to help build small learning communities within their institutions.

Student Ambassadors will be encouraged to:
• Facilitate informal clinical and diagnostic discussions with fellow students
• Introduce colleagues to evidence-based educational resources
• Discuss clinical cases, diagnostic reasoning, and treatment planning
• Bring student questions, perspectives, and educational feedback to the Academy
• Connect with Student Ambassadors from other dental schools and countries
• Participate periodically in international educational discussions with Professor Marwan Abou-Rass and the Academy

Ambassadors are not expected to be teachers or experts.

They are students helping other students question, discuss, think, and learn.

Selected Student Ambassadors will receive complimentary access to the AbouRass Endodontics Academy. Other participating students will be eligible for a special student-access program.

We welcome applications from motivated dental students and recent dental graduates worldwide who believe that dentistry must be built on accurate diagnosis, scientific evidence, biological understanding, and sound clinical reasoning.
Our principle is simple:

Learn locally.
Discuss collaboratively.
Connect globally.

THE COMPLETE 4ROD PROTOCOL IS NOW PUBLISHED

I am pleased to announce the publication of the final article in our four-part interview series:

The 4R Operational Diagnosis Protocol: Part 4—R4: Restorative and Tooth Structure Assessment

R4 addresses a frequently overlooked reality: endodontic diagnosis cannot be completed by evaluating only the pulp and periapical tissues.

The clinician must also assess the existing restorations and the remaining tooth structure—including restorative quality, cracks, discoloration, tooth wear, developmental anomalies, alignment, and occlusion. These findings may explain the origin of disease, influence treatment decisions, and determine the tooth’s long-term prognosis.

With the publication of R4, the complete 4ROD framework is now available to the dental profession:

R1—Patient Report and Rapport
www.genesispub.org/resource/images/articles/pdf755…
R2—Radiographic Findings
manuscriptscientific.com/images/2ddd2976aac44b31e5…
R3—Response Testing
manuscriptscientific.com/images/17fd69979925a4b1b5…
R4—Restorative and Tooth Structure Assessment
manuscriptscientific.com/images/7e750d1dcbcf67c8dd…

Together, the 4Rs provide a structured, evidence-based approach that moves diagnosis beyond the traditional pulpal and periapical labels toward comprehensive, patient-centered decision-making.

My sincere appreciation to Deeraj Sharma and the Journal of Oral Health and Dentistry Research for providing the opportunity to present this complete diagnostic philosophy to the profession.
Diagnosis is not the collection of isolated findings. It is the disciplined integration of evidence.

The 4ROD Protocol – Part 3 Is Now Published

For decades, endodontic diagnosis has focused on performing tests. Perhaps it is time to focus on interpreting biological responses.
I am pleased to announce the publication of Part 3 of the 4R Operational Diagnosis Protocol:
Mastering R3 – Pulp and Perio Response Testing

Read Here: manuscriptscientific.com/images/17fd69979925a4b1b5…

This article discusses why response testing should extend beyond traditional pulp testing and presents a biologically based approach that integrates:
• Patient findings (R1)
• Radiographic interpretation (R2)
• Response testing (R3)
• Restorative evaluation (R4)
Topics include:
• Why I rely primarily on the Cold Pulp Test
• The biological limitations of Electric and Heat Pulp Testing
• The 5×5 Periodontal Rule
• Isolated Periodontal Pockets
• Sinus tract tracing
• Why tests alone never establish a diagnosis
For readers new to the 4ROD Protocol, links to the previously published R1 and R2 interviews are included within this publication to provide the complete background.
Diagnosis is not about collecting tests. It is about interpreting evidence.
I look forward to your thoughts and discussion.

Marwan Abourass

After decades of clinical practice, teaching, research, and continuous refinement, I am pleased to announce the first published release of the 4R Operational Diagnosis (4ROD) Protocol.

This publication represents much more than two articles.
It introduces a comprehensive, structured diagnostic framework to guide clinicians in collecting, interpreting, and integrating diagnostic information before treatment begins.

The protocol consists of four sequential components:
• R1 – Patient Report & Rapport: transforming the patient's story into organized clinical data through structured communication and examination.
• R2 – Radiographic Findings: introducing the 10 Areas of Diagnostic Interest (10 ADI), a systematic approach to interpreting periapical radiographs.
Future publications will continue with:
• R3 – Response Testing
• R4 – Restorative & Structural Assessment

My sincere hope is that this protocol will stimulate discussion, encourage critical thinking, and contribute to more comprehensive, biologically based diagnoses in endodontics.
I would like to thank the editors and publishers for providing the opportunity to introduce this work to the profession.

Read Part 1 (R1):

Read Part 2 (R2):

I look forward to your comments and professional feedback as the complete 4ROD Protocol continues to be released.

PPPQ - Structured Diagnostic Tool for Orofacial Pain Assessment

The PPPQ was developed during AbouRass's advanced studies at the University of Pittsburgh and has been refined over five decades. The PPPQ transforms the patient pain story assessment into a structured, patient-centered diagnostic tool. It gives the clinician insight into the patient’s pain behavior, patterns, triggers, and psychosocial factors. It is based on verbal descriptors. It is more informative than visual analog scales (VAS), verbal rating scales (VRS), or numeric rating scales (NRS). When combined with the 4ROD protocol, the PPPQ becomes an indispensable clinical asset.

To my colleagues and followers,

Over the coming weeks, I will begin releasing short video presentations for each PowerPoint lecture in the AbouRass Endodontics Academy Master Class.

I will start with the most important subject in dentistry. And that is, Diagnosis—not technology—is the most neglected skill in clinical dentistry education.

The releases will follow a clinical diagnostic sequence, reflecting how diagnosis should occur in daily practice.

Each video will introduce the why, the clinical relevance, and how it fits into real-world decision-making.

Full presentations and structured learning are available at:
