There’s a mindset in manual therapy that a manipulation needs to be delivered with maximum force to ā€œwork.ā€

It doesn’t.

Mobilisation and manipulation may produce overlapping neurophysiological effects, one is simply delivered at a lower velocity than the other. Manipulation doesn’t require brutality to be effective. It requires an appropriate amount of force for that patient, that tissue, that moment.

The research is clear on this much: applying more force produces more measurable force in the soft tissue and surrounding structures, more vertebral movement.

But that’s not the same as a better outcome.

No trial to date has shown that increasing force improves how a patient actually does.

There’s no universal ā€œcorrectā€ force either. Patient size, tissue tolerance, age and presentation all vary, so there’s no formula to chase and no justification for going in harder than the joint and the patient need.

Excessive, uncontrolled force doesn’t make a technique more effective. It increases discomfort, undermines patient trust, and has no evidence behind it as a driver of better results.

For context, average thoracic thrust force in published research sits at ~124N is equivalent to the weight of 12.6kg. Far less than most people assume.

Use what’s necessary. Not maximal. Not brutal. Necessary.

Skill and clinical reasoning beat brute force, every time.
