Knee Plica Syndrome
Definition:
Knee plica syndrome is a painful condition caused by irritation or inflammation of a remnant synovial fold (plica) within the knee joint. Plicae are normal embryological structures that usually regress, but sometimes persist and become symptomatic.
Anatomy & Types of Plica:
There are four main types of synovial plica in the knee:
Suprapatellar plica – above the patella.
Mediopatellar (medial) plica – along the medial aspect of the knee, most commonly symptomatic.
Infrapatellar plica – below the patella, also called ligamentum mucosum.
Lateral plica – rare.
Etiology / Pathophysiology:
Normally asymptomatic, but can become pathologic when:
Trauma (direct blow or twisting injury)
Overuse (repetitive knee flexion/extension, common in athletes)
Inflammation due to maltracking of the patella
Thickening or fibrosis of the plica causing it to rub against femoral condyle or patella
Clinical Features:
Symptoms:
Anterior or anteromedial knee pain, worse with activity (stairs, squatting, running).
Clicking, snapping, or catching sensation.
Intermittent swelling or stiffness.
Signs:
Localized tenderness over medial femoral condyle.
Palpable cord-like band sometimes felt during knee movement.
Pain reproduced by flexion-extension with internal rotation of tibia.
Differential Diagnosis:
Patellofemoral pain syndrome
Meniscal tears (especially medial)
Chondromalacia patellae
Osteochondritis dissecans
Investigations:
Clinical diagnosis is most important.
MRI: May show thickened plica or exclude other causes.
Arthroscopy: Gold standard for both diagnosis and treatment.
Management:
Conservative:
Rest, activity modification
NSAIDs, ice application
Physiotherapy (quadriceps & hamstring stretching, strengthening, patellar tracking exercises)
Corticosteroid injection (in resistant cases)
Surgical:
Arthroscopic resection/excision of the symptomatic plica if conservative measures fail.
Prognosis:
Most patients respond well to conservative management.
Arthroscopic excision gives good results with low recurrence if symptoms persist.
