For PT and OT owners: are you budgeting 2026 off the +3.26% conversion factor headline, or off your actual net?

Asking because the two are pretty different this year. Yes, after five straight years of cuts, CMS finally raised the Medicare conversion factor, from $32.3465 to $33.4009 for most outpatient PT/OT (non-qualifying APM), a 3.26% increase; qualifying APM participants get $33.5675 (+3.77%). Genuinely good news. But in the same rule, CMS finalized a permanent −2.5% efficiency adjustment to work RVUs on non-time-based services, which hits evaluation and re-evaluation codes (97161-97164).

Timed treatment codes are exempt, so the net depends heavily on your code mix. A professional association's analysis pegs the real net around +1.75% for most PT practices, and eval-heavy clinics can land near flat. On top of that, a big chunk of the increase came from a one-time statutory update that may not carry into 2027.

So the practical playbook: model your own code mix (not the headline), and grow revenue that doesn't depend on the fee schedule at all, RTM being the obvious fit since it reimburses monitoring many of us already do for free. What makes this urgent: emergency medicine has the highest burnout rate of any specialty, and bureaucratic/EHR workload are the top two burnout drivers nationally. So documentation time is a design problem, not a personal failing, you can't ask clinicians to type faster.
